TERMS OF SERVICE AGREEMENT:
This segment represents an agreement and understanding between Chair Massage In Dallas Tx and the below signed person/Company (hereinafter called CUSTOMER)
CUSTOMER understands that Chair Massage In Dallas Tx requires Professional Liability Insurance Coverage for each of its subcontractors/independent contractors and will provide detailed policy information & verification of said coverage to CUSTOMER upon request.
CUSTOMER agrees, to indemnify and hold Chair Massage In Dallas Tx, harmless from any claim, loss, cost, expense, demand, or damage, arising directly or indirectly out of the use of its on-site services and activities in connection therewith.
CUSTOMER agrees not to solicit agents, independent contractors or representatives of Chair Massage In Dallas Tx directly or indirectly for employment, contract or otherwise.
Pay via Credit Card: For the initial engagement, CUSTOMER agrees to supply a valid credit card to secure the scheduling/booking and to submit payment on or before the date of service. A 3.5% swipe fee will be applied in order to use your credit card.
Pay via Check: We require payment in full no later than 3 business days after the event. If you choose this method, we’ll send you an invoice to submit for full payment as to have it ready right after the event. Check must be mailed via a mail carrier with proof of delivery/tracking services.
Affordable Rates – Our rates for massage vary depending on volume, number of hours and number of therapists requested. You may pay a higher rate for weekends. Rates may be lower for very high volume contracts. And rates may be slightly higher for short-notice events or for events lasting less than 2-hours.
Best Value – The easiest way to afford chair massage and lock in the best rate is to schedule multiple massage services on a regular basis. That’s because we can give great discounts when you schedule higher volumes of massage.
Parking must be validated by you for each Therapist/Provider.
No refunds will be provided for deposits or cancellations received less than 24 hours before a scheduled appointment. Cancellations received between 72 hours and 48 hours before a scheduled appointment are eligible for a 75% refund, and cancellations received less than 48 hours before a scheduled appointment are eligible for a 50% refund. Additionally, events with 5 or more therapists/providers that have been booked and confirmed, will incur a 10% non-refundable fee for any cancellation, at any time.
The Terms of Service and the relationship between CUSTOMER and Chair Massage In Dallas Tx shall be governed by the laws of the State of Texas without regard to its conflict of law provisions. Customer and Chair Massage In Dallas Tx agree to submit to the personal and exclusive jurisdiction of the courts located within the state of Texas.
**Note: Please be aware that any session or event starting at or after 6pm or ending after 8pm or on/near a major holiday or on the weekends will incur additional fee's of $25+ per Provider/per hour. Any business establishment or residential home/area that requires Provider/s to enter or exit stairways/steps (w/no elevator access) will incur an additional $25 charge per Provider.**
By submitting this form, I am representing myself as an agent of said company/Customer with the authority to agree to these terms of service.
